Each spinal nerve provides sensation to different regions of the skin (dermatomes). The S1 dermatome includes the lateral margin of the foot and little toe. Foot pain secondary to S1 is radicular pain from an injury/abnormality occurring at S1. This pain is often described as numb, burning, or stinging.

It could be that a nerve is being hit from a protruding disc. When that happens a lot of times it will send pain down the leg and some people feel the most pain in the foot. Some will experience numbness as well at times. You might want to get an x-ray and an MRI.

If this disc is slipped, herniated, or somehow injured in another way, it can and likely will pinch a nerve that goes to your foot. This nerve is called a nerve root. You may need an evaluation by your physician to see if this is indeed the cause of the foot pain.

The S1 is a heart sound, signifying systole, or contraction of the heart muscle. Since your heart is not closely related to your foot, there is no real connection between foot pain and the S1 sound.
