How often do employers contact past employers for a reference?

Always. They want to know the work history and if they would rehire you again. There is a limit on what the previous employer can say. Yes, you worked there and I would or wouldn't rehire you, and that's about all.

It varies tremendously with the employer, the level of responsibility for the new job, and the industry. Larger employers are more likely to check references, and you are also more likely to have them checked for any senior position. It is always possible that any employer will check, however.

On your application you are asked to list your former employees. References aren't generally given by former employees because of legal issues. Past employees are just contacted for factual information, such as title, pay and length of service.
