First, I'd make sure that you really do want to get back together with him and evaluate your reasons for ending the relationship in the first place. Then, be open and honest with him, but be prepared that he may not want to re-enter the relationship.

You want him back probably because you feel alone and insecure, but if the relationship was incompatible, it will not ever work anyways, so you must stay strong and move on
