To collect short term disability benefits that are provided through your employer consult your benefits coordinator to verify your eligibility and request assistance to make a claim. The benefits coordinator will provide you with instruction and necessary forms to begin the process. If you are incapacitated, a family member should contact the benefits coordinator for you.

Find out the kind of short term coverage you have. Some companies require you to have worked for a specific amount of time before they will cover you. Contact your HR representitive for the forms needed. Take the forms to your doctor and let him release your records. Take your forms back to HR and they will complete the process.

The only short term disability programs in the United States are those offered by private insurers; Social Security does not offer short term disability at all. The exact requirements, time frames and procedures vary with the insurer. Commonly, you will have to provide extensive medical documentation and be seen by an outside doctor.

When collecting short term disability at your place of employment, you will need to obtain all the forms needed to start. Once you have obtained these, see a doctor to get an official statement on your reason for being put on disability. Once all this is done and paperwork is filled out give it to your HR department to complete.

Short term disability is typically provided by your employer as a standard or optional policy. Claiming it generally as simple as filing a claim with the insurance company (ex. AFLAC, or Standard Insurance). Your Human Resources Person should have all of the paperwork. For certain types of illnesses like pregnancy, the payments are almost automatic.
