If you are a taxpayer or a sole-proprietorship you put your social security number and other info on the form and send it back to the requester.

Many places require a W-9 to be completed before they will pay for work. If you have a FEIN you will would use this number, otherwise put your SSN on it. Complete the rest of the information and return it to them. Always keep a copy for your records too.

Fill out your name and address, then give it to the requestor (typically a financial institution). Without this form, the institution is required to withhold taxes for the IRS.
