Any guinea pig with a bald spot should be checked for mites. There are mite treatments that your vet can give you to get rid of them.

If it is the front paw, it could be from wiping her nose. Could also be mites or a fungal infection. Take her to an exotics vet for a checkup.

Hold the guinea pig in one hand under a bright light and use a magnifying glass to check for a splinter. Maybe the guinea pig is trying to eliminate one.

It might have a infection in that area. Or he could have a mange problem. In any case call your vet.
