A common cause is a bacterium called Canine Brucellosis. This disease is common in kennels or in situations where multiple dogs reside because it is so easily transmitted.

The same as for humans: genetic problems, malnourishment, injury, size of puppy (too large or too small), poor care, too much exercise.

Stillbirth can happen if a litter is too large for the dog to handle. Another cause can be premature birth. With multiple mating, gestation can be three weeks apart.

It depends on a lot of factors. Sometimes the mom didn't get enough nutrients, sometimes the puppies were in the birth canal too long, etc.
