There are several possibilities, including a problem with the adrenal gland. These problems would include an adrenocortical tumor, adrenal cancer, or adrenal hyperplasia. DHEA is also elevated in 50% of women with polycystic ovarian syndrome. If a woman has elevated DHEA, she should see her doctor, who should look at other symptoms before deciding how to proceed.

DHEA can be elevated by regular exercise. It is also thought that calorie restriction will also produce its increase. The most common cause is intentional ingestion for training purposes.
