To file for a 485 search the immihelp website for a fillable application. Along with the form you also need one thousand and fifty dollars to cover the application and EAD.

You can get the form from the U.S Citizenship and Immigration Services Centers. Once they have recieved the form, they will do a background check before your application is accepted.

You need to file a I-539 form, Extension of Stay/Change of Status. You need to copy your records (passport, proof of financial report). Get the form along with the address from www. immihelp. com/visas. Where you send the form depends on what state you are residing in.
