chlamydia caused sexually transmitted diseases in human, so they are transmitted via body fluids. Its occurs through anal mucosa, oral sex and vaginal sex. It can also be transmitted from an infected pregnant mother to her baby during the delivery. As the disease progression can be very slow, the infected partner can transmit it to the healthy one.

Well, like any other secually transmitted disease, it is mainly caused by not knowing that you had it, it got sporatically worse over time, and you continued to have unprotected sex anyway.
