The best back massager that I have ever used was a real massusse in a real Spa. Next to that there are many electronic back massage units available for you to use. Just because it is expensive does not mean it is the best but if I had to choose it would be The Sharper Images All-in-one massager chair.

Another human being, mechanical back massagers cannot compete with the hands of a skilled masseuse. They can feel where your back is tight and know the appropriate technique to apply.

I think the best at-home back massager is a simple tennis ball. All you need to do is put the tennis ball on the floor (I usually put a towel down first) and then lie down on top of the ball and move your back over the ball. You have the control and can adjust pressure and location.

The best massager depends on where it hurts on your back and why. If it's sore muscles a warm shower can help or a soak in a warm tub.
