The life insurance company will generally require a physical as well as blood work. They may ask for additional tests. The tests are paid for by the insurance company.

Any tests depend on the amount of coverage you're applying for and the expenses are paid by the insurer. A rule of thumb is that any policy under $100K will not require any tests but merely questions and health record review. Over that amount is blood testing and the like.
