Usually the term is distended meaning that the bladder is not draining properly causing urinary stasis. This can lead to bladder infections. So yes having what you called an extended bladder can be harmful

Bladder is part of the urinary system. It is an organ that holds the urine. It is like a receptacle that contains all the water and waste that our body no longer needs. This condition is not normal because it signals that all toxic substances are piled up and the bladder is so full that it needs catheterization.

Extended bladder is when the bladder itself becomes weak due to medical procedure on the prostate. Sometimes a catheter is used to help the bladder rebuild.
