An adult birthday cake can include anything the birthday person is intrested in. Cars or golf for men. Purses or shoes for women. Or just flowers or plain candles.

To be anti-"adult" why not go with a throwback themed cake. If they were born inthe 80's pick something like He Man or Strawberry Shortcake. Or if they were born in the 60's maybe do a Batman cake. They are always good for a laugh, and make us nostalgic.
