There are a lot of factors that are part of the consideration of whether to do HRT. Each of those factors is considered by the physician, who will discuss the related issues with each patient. Age, history and reason for the surgery are key factors.

In the largest clinical trial so far on HRT, only 8 women out of 10,000 came down with breast cancer, and 33 of 10,000 experienced vascular problems such as abnormal blood clotting, stroke, or heart disease. However, HRT can mitigate menopausal symptoms, colorectal cancer, and bone loss.

HRT after a hysterectomy is the common mode of treatment if an oophorectomy is also completed. A link has been found between post-menopausal woman's HRT and increased risk of cancer, however the women studied still had their ovaries. However, usually for a hysterectomy alone women do not need any HRT.

HRT is fine after a hysterectomy as long as you don't smoke or have a history of blood clots. Cancer isn't an issue because the uterus and ovaries are gone (you can't have uterine or endometrial cancer if you don't have a uterus!). Unless you've had a stroke, DVT, or PE, you should be ok.
