Cold sores usually do not drastically swell. Excessive swelling can be a sign of a deep-tissue injury, such as from a traumatic injury to that portion of the body. These types of injuries cause trauma to the sub-dermal tissues and result in swelling as a protective mechanism against bacterial invasion.

Cold sores are often red, inflamed, swollen, and painful. Cold sores should resolve on their own within 1-2 weeks. If the area of redness spreads, consider seeing a doctor.
