Before you write a demand letter make sure you have proof, witnesses and you reported it before you left the store. Also have a doctor's note and hospitals bills.

Address your letter to the manager by name. Clearly and concisely detail when and where your fall took place; what happened after your fall; a verifiable account of your injuries; and a clear request for what you want the store to do.
