You must complete an application for a replacement card. The application is two pages; the instructions are four pages. The application and required supporting documentation, along with $290 fee, should be sent to USCIS, P.O. Box 21262, Phoenix, AZ 85036. The payment must be drawn on a US bank and payable in US funds.
