I would not recommend doing this without an attorney. At least check Legal Zoom which will give you the forms and ask you the right questions. No guarantee.

I believe you have to state what items are part of an estate that you lay claim to. You will not need a lawyer but it will usually need witnesses and you may have to go to a bank or a government office and for a small fee have it officially notarized by a person licensed as a Notary Public.
