For the letter/envelope: The Honorable Mary Jones, (line 2:) Judge, Family Court, First District, (line 2/3) rest of address. Start the letter: Dear Judge Jones . . .

When writing to a judge, assuming that it is not for a court filing, you address him as "Judge" so-and-so. Thus, if his last name is Smith, use "Judge Smith."
