You may request a copy of a tax transcript directly from the IRS by filling out Form 4506T via mail. If you want to request it by phone, you may call the 800-829-1040 and request a copy also. The tax transcript for this year and prior 3 years will show exactly how and what you filed.
A couple ways, you can contact your local municipal court, or you can call up an attorney or personal clerical administrator, they'll have the information you need to obtain documents.
