First speak to your parents about it and if they are in agreement hire an attorney to legally get your name changed.

The minor would have to file a form at the county clerks office. There has to be a reason for the name change and the minor may be required to appear before a judge before the name change can take effect.

We've actually had a daughter do this when she was 16. She went and got the paperwork to be filled out for name change, took it to the courthouse and paid the filling fee's, this was either accepted or rejected, in her case it was accepted. And she had a new legal last name.
