When my cat suffered from this, the biggest indicator of her problem was repeated trips to the litter box. She would try to urinate each time, without much results. She also acted quite sick, but she also seemed uncomfortable and restless.

One of my cats has had many urinary tract infections. You will notice your cat trying to urinate frequently regardless of whether he really needs to or not. That is because the infection makes him have the frequent urge to urinate. He may even try to urinate outside the litter box.
