You can use google doc to create a spreadsheet. There you can add a number of types of questions, like multiple choices, check boxes, radio buttons, or text answers. When you send the links to your friends you can see the result there.

Brainstorm all the areas that you need covered. From there formulate questions that can give you the information you need. Send it out in an email, or photocopy it and hand it out.
