Well start by writing what questions you want to enclose in the questionnaire (making sure to keep the topic relatively specific so you don't end up with overly broad answers. Edit for gender specificities, and be careful of putting too many cultural references in case there are those who don't understand them.

Well, basically you write questions, and leave space for people to answer them. Decide on what sort of questions you want to ask, and what format you want to have people answer in, multiple choice, fill in the blank, true/false, etc.

First I would start being making a list of the information that I needed to know. I would then formulate questions that were concise and simple. If you are trying to keep your results within certain responses, use multiple choice questions. Also use a simple 1-5 scale for rating answers.

First, understand the subject at hand and do a complete history and research of it. If you know what kind of answers that you seek, have your questions revolve around your predicted answers. If not, improvise until something exciting and ineresting develops.

Plan the questionnaire carefully. Make the questions clear and concise, and use a simple answer system like 1-5, or "never, sometimes, always." If you want to solicit opinions, include a space at the bottom on on the back side of the questionnaire for more elaborate comments. Keep the questionnaire anonymous so that participants feel free to be honest.

Gather information for the questionnaire you whant to write. Keep the questionnaire as short as possible. Use a style that is easy to understand for anyone. Use multiple-choice questions. Avoid leading questions, because they might generate false positive responses. Keep the same rating scales for all the questions. Test the questionnaire on about 10 people.
