Does my employer, by law, have to send me regular 401k statements?

You do not receive 401K statements from your employer, they should come from the investment firm that the 401K is held with. By law they only have to provide annual statements, but you may be able to request a statement on a quarterly basis. Do be aware that the administrators are in no way legally obliged to do so.

Your employer does not send you 401K statements. The investment firm which holds the 401K statements must send statements at least quarterly, but can send it monthly. The employer does have to state how much of your check has been sent the 401K.

Your employer should not be the one sending the statements. There should be a third party plan administrator that sends statements. Current legislation requires you only to receive an annual statement. Quarterly statements are not mandatory. You may want to inquire as to if there is anywhere online where you can view your 401K statement.

No. The investment firm that holds the 401K must provide statements on at least a quarterly basis. And the payroll department must list all deductions to the 401K on each pay stub.
