The 220 volt disconnect should be wired by a cerified electrician. I'm guessing, if you live in any central area, you are violating local codes. There is a schematic, that if you were an electrician, on the inside of the door of the disconnect which explains readily how to hook it up.

It IS better to install a sub panel with 2 pole breaker.for electric tankless water heater. It requires sub panels for every 50 A /240 V feed

You will need a contractor, a tankless system will require the modification of the energy system in your home, unless you are a contractor or an electrician, you will need professional help.
