Medicare is a federal program. When you arrive in your new state, simply put in a change of address with the local Social Security Office. Your Medicare should cover your medical needs in all 50 states, even when traveling.

Yes, you can always do that. Contact Medicare customer support and tell them that you are relocating and ask them to move your medicare insurance to your new place. They will raise a ticket for you.
