While indeed oxygen is free, there is a difference in the quality of the oxygen around you and the oxygen that is bottled. The bottled oxygen is often free from chemicals and bacteria and so on. It is pure and that is what is commonly used for those who have problems breathing and need pure oxygen.

People who buy bottled oxygen usually have respiratory problems and cannot easily breathe the oxygen in the air. Usually people who buy oxygen put it on their back in a pack and then have tubes entering their nostrils helping them breathe.
