This is done by going to your local Social Security Administration building. You will need to show legal proof of the name change.

You can file the paperwork either online or at the Social Security Office. It's actually quicker to print the paperwork and mail it in. If you got married you will need a copy of your marriage license. They will contact you if they need any extra paperwork or identification items.
