If there is a patent on the logo, you cannot reproduce it. Like anything Disney. If its your own logo then take a copy to Kinko's, tell them what you want and they can do it. It will cost you.

There are several options online. If you go to a website like vista print you can upload your logo and get the notepad mailed to you. You can also get this done at some office supply stores.

Local printers can usually make these for you. Check your Yellow Pages. You can also check with your local FedEx/Kinko's store. Some of the larger ones have this capability, and if they don't, they are usually knowlegible of where you can get it done. Online companies such as Vista Print also provide this service.

around here you could get that done at office max or any novelty book store. you could also try googling that. im sure you could buy them online. try that.

There are many good websites to make them. Design your own logo - don't pay someone else to do it. Then, upload your image and have the pads made.

the best thing to do is shop around and ask people at a computer store or a graphics design store if not you can always shop online but always shop around
