This could be caused by a severe chemical or allergic reaction to some type of hair product used. It could also be indicative of a more serious problem such as an autoimmune disease, Grave's Disease, or other diseases similar in nature. There is also a possibility that it could be stress related.

This could be a symptom of alopecia. This is rapid hair loss due to a disease of sorts that would cause all of your hair to fall out, a common symptom after rapid hair loss is a burning and/or itching sensation around the scalp. Over the counter remedies may not provide complete relief.

You may have a condition called Trichodynia. The pain is actually caused after the hair is lost. The best way to treat this condition is to prevent hair loss.

Most of the time this is due to some sort of injury, although most commonly found after bleaching and straightening of hair.

It could be any number of things you really should call your doctors office and set up a consult with them so you can go in and explain your symptoms and let them help you come to a diagnoses and treatment plan any other way you do it would be just a guess and that could be very dangerous.
