Well basically you have to include all of the facts that you would like to dispute. You need to prove why you are disputing and the reasons and facts for dispute should be very clear and persuasive. If they are not persuasive you may not be able to dispute effectively.

Respondent request relief that a judgment rendered against you was not properly served, or the court did not have jurisdiction over that type of case. One could also state, a person named or served was not originally a party named in the suit. There are disputed material facts sufficient; as the judgment was fraudulently altered to increase the amount granted.

Seek the advice of an attorney. Any response must be done quickly and accurately or you risk losing your case and the Judge finding in favor of the other party. A response should follow the format of the motion and respond basically paragraph for paragraph with a basic theme that facts are in question, disagreed on, and disputed.
