What should be included in a Confirmation of Employment letter?

I'm ask to provide one about once a month. I don't know what to say.

The new employer can inquire (with permission of the applicant) of the former employer to verify the employee's claims of previous employment. As a former employer, you may only state the individual's name and the start and end dates of the individual's employment. You may not include commentary or any other information.

It should include the employees name, position, salary, pay schedule (weekly, biweekly),if they are full time or part time, and contact information for you or your business. These letters are generally from land lords or state agencies seeking to insure that tenants/clients have not lied about their employment or salary information.
