Be honest first of all, and be thorough and straightforward in your explanation of why you are unable to pay a debt. If possible, offer an alternative time frame for paying so they'll know you're sincere.

It should include current income, including unemployment and disability. It should include all required payments, including child support, debt payments, and taxes. It should include the number of people who depend on this income, including spouses and children. It should list a reasonable budget to meet the basics of life - and why you cannot meet their desired financial obligations.

Everything! Depending on why the court is requiring this would depend on the content but you would include the amount of your personal bills and anything of necessity.

The debtor should explain the circumstances that brought him or her to this point, a Court Hearing. For example, has the debtor been sick and therefore without income? Has the debtor's former partner failed to pay child support for years? Has the debtor been unable to find work?
