What is going on if I can't tell if I have a toothache or an ear ache?

I'd say that is probably your lymph nodes aching. They are just below your neck and close to your jaws. It is like part of your immune system fighting against sickness.

This is from an inner ear infection in the eaustation tube. Swelling in the tube puts pressure on your jaw.

The nerves of the ear and the gums/teeth are connected. It's probably a toothache that has displaced the pain to the ear as well. It's not likely that an ear ache would cause the tooth to hurt unless you have a severe ear infection that has spread. Start with the Dentist and go from there.

You could have an impacted molar or wisdom tooth. An impacted tooth that is far back in your mouth close to your ears may make it hard to determine if it is a tooth or an ear infection. It probably would be best to see a doctor about the problem.
