The acronym "BIRAD" stands for "Breast Imaging Reporting and Data Systems." There are five BIRAD levels which doctors use to classify risk. Since level 5 is the highest, BIRAD 4 is indicative of malignancy suspicion.

After a radiologist interprets the output from a mammogram, he / she will use a scale, the birad scale, from one to six to categorize the results; hence, a birad 4 means that a suspicious abnormality was shown, which means there is a higher probability of cancer.

BI-RADS 4 is a rating given to a patient after having a mammogram completed. The BI-RAD 4 means that there was something suspicious and that there is a 23-34% chance that it may be breast cancer. A biopsy of small tissue will be needed in order to make an actual diagnosis.
