Every, single thing with regard to the lease should be on the lease. All details must be contained "within the four corners" of the lease in order to be valid. Verbal agreements between tenant and landlord are not valid unless they are contained in the lease. Everything from lawn care to utilities to pets to late fees--everything must be included.
The release form should contain applicant's social security number, and bank account information, also present and passed address information, also income or job status, date of birth, contact information.
