What happens that causes a scar to form at the site of an injury?

The scar is always a different color than the skin around it.

It's my understanding that the scar is fibrous repair tissue. Since it didn't grow through the usual skin method (epidermis, dermis, subdermis), it looks different.

A scar is the result of the healing process. The body produces collagen to heal the wound. Some individuals who produce more collagen can end up with larger scars.

The bodies repair mechanism requires fibrobalst to lay down collagen and fibrous connective tissue. Unfortunately, it is of darker pigment, but will fade as remodled over time.

The skin cells themselves have the ability of rejuvernating and it is the protection action of most cases that area is also itchy when cells grow.Having more vitamin C will help to shorten the overall process and cells will then grow quickly.Be sure to keep that area clean in order not to infect.
