Kittens "knead" their mother as they suckle to help the milk flow more freely. It is instinctual. As adults, some cats continue this behavior. It is thought to be similar to purring. Cats will "knead" a favorite blanket, bed, or even you when they are happy.

When a kitten is kneading it's mother when nursing it is actually kneading her nipples to stimulate the let down reflex of her milk.
