What do I need to do to replace a Social Security card?

I laminated mine and I need to get a new one.

You will need a valid ID, such as a state issued driver's license, and complete form SS-5 provided by the Social Security Administration. You might also need to bring in your birth certificate or naturalization papers to prove US residency if you haven't before.

You will need to go either go in person to a social security office or mail in an application you can print from the Social Security Administration's website.

Look in your phone book under U S Government, find your local Social Security Administration office. Go in, request a replacement card. Walk out with it in hand.

Depending on where you live, contact your social security office and ask about documents needed for new social security card. Gather the info and make an appointment with the social security office. some states require advanced notice, others do not. Again, it depends on where you live. Here in michigan, you have to call ahead and speak with a processor.
