The nature of the swelling should be established.Thyroid function test is needed for thyroid status. Ultrasound scan and tissue biopsy will be indicated in certain case to exclude possible malignancy.

It really depends on why the thyroid is swollen. The treatment can be as simple as iodine or as invasive as thyroid surgery. See your doctor.
