I've seen ones made of old children's cloths quilted into the shape of the children by tracing their bodies. Another way is photo transfer onto cloth and sew them in.

Printer fabric is available at craft and office supply stores that allows you to print your favorite family digital photos directly on fabric. Once you have chosen a quilting pattern, simply substitute the photo panels throughout the design
