What are some unique designs for a lower back tattoo?

A "scene" of some kind as from a famous painting, or a book or movie title would work well as the lower back area (where the pant / skirt top would lay) is flat and would lend itself well to a scene as if characters are walking and in action.

When getting a tattoo, you need to pick out something that has a sentimental meaning to you, not just something that is unique. This piece of artwork will be with you for the rest of your life. You need to consider what colours (if any) will look best against your skin and the tattoo should flow nicely with your curves.

Lower back tattoos are usually something symmetrical so you could go for an artpiece like a painting or drawing that is assymtrical, or perhaps a writing across the back. Also, something not horizontal even would be different.

A tattoo that looks like a belt is a very unique and interesting lower back tattoo. The belt can have notches representing goals or just fun charms hanging off it.

There are so many different themes you could choose from. Flowers and vines are always classic. Of course you would incorperate your favorite flowers. Butterflies and dragonflies surrounded by pretty vines or flowers. Tribal is also nice because the designs are always so different. Stars and moons for a celestial theme or maybe a heart surrounded by ribbons.

Depending on your body shape, a taller or wider tattoo will flatter your features. Go with something that either expresses an interest of yours, or is connected with your personality.
