Ask her/him to tell a story about their worst first date (but be prepared to tell yours, too). Tell a story about a crazy teacher you had in school, and then ask her if she has any similar stories. Ask about their favorite comedy (movie or tv) and then try to recall funny episodes.

To keep things relaxed, best to stick to easy-to-answer questions rather than funny ones. The latter can put unnecessary pressure on the other person, if they are nervous and not naturally humorous or creative.

Ask the question, How long have you been single and lonely? Is that really how you you chew your food. And what is that hanging from your front teeth. How ya like me know.
