Should I be concerned about a puppy that has chronic UTI's?

My puppy has had four UTI's in the past five months, I am concerned that something worse could be going on.

Repeated urinary tract infections are a red flag that something else is going wrong in the body. For example, if the immune system is being suppressed due to drugs or another problem, the body will be more susceptible to any infection. You should talk to your vet about this problem.

Maybe you are not letting your puppy outside enough to use the bathroom. Some puppies can only hold their urine for so long, just like humans. Try letting your puppy outside more. If the problem continues, consult a vet to see if there is something going on in his/her urinary tract.

While something else could be going on, try changing his dog food. Sometimes this will eliminate the problem, if not have the vet to screen him for other problems.

Yes, you should be concerned about a puppy that has chronic UTI's because Chronic UTI's can be related to kidney problems. The UTI's are a likely a symptom of the problem, rather than the root of it.
