You can have a toenail removed but mostly people have parts of the toenail removed to prevent ingrown toenails. It is very painful but not very expensive.

Yes but it's also possible that the nails grow back. Mine grew back from all four sides, grow towards center and overlap. Minute possibility for that to happen though.
