If you were sick as a dog and out for three days, a doctor's not might make it easier for your colleagues to believe you were actually sick.

If you were that sick, I would wonder why you didn't go to the doctor in the first place. Though it all depends on the company's policy that you work for. You were probably explained all this when you started the job. You could go to the doctor on the 4th day and have him write a note for you.

Well, I think if you can prove that you were sick without a doctor's note you don't need one, You should go to work and ask if your Boss asks you for one just say you left it home and would bring it with you the next day. The next go to the doctor and ask for one.

Yes, defenitely you need a doctors note. Now a days small fever is the symptom for a big disease. Any how you have to consult a doctor and take the necessary action.
