If you put it back in the refrigerator asap you should be OK. Once the milk reaches a temperature over 100 degrees it becomes a salmonella and bacteria playpen. A good test is to touch the container and see if it is still on the cool side. But, when in doubt - throw it out.

If the lid was on it and it wasn't hot in the house it should be okay. If it smells funny though throw it out.

Generally speaking bacteria takes about two hours at room temperature to cultivate, what this means is wehn milk hits room temp,it takes two hours to go bad.
