How do you write a sixty day notice?

I need to give a sixty day notice in writing to my landlord and have no idea how to start the letter.

Your 60 day notice does not have to be fancy. This is to inform you that i intend to move out of my apartment on mm/dd/yy. Make sure to sign the notice and date it and then have the landlord sign it that they received it and keep a copy.

Dear Landlord (or name), This letter is to inform you that I will be moving from the (name of) building at the end of (month, and I am now giving you 60 days notice as required by the terms of the lease. etc...
